Vital Records

Vital Statistics are not public records, and availability is restricted (as noted).  You may obtain a record by providing the specified identification and meeting the requirements. 

In person:
Bring the appropriate identification for the type of certificate you are requesting, along with $10 fee (either cash or money order), and a completed application (which you can download from this page) or which will be provided when you come in to the office.

By mail:
Enclose a photocopy of the appropriate identification for the type of certificate you are requesting,  the required fee of $10.00 per certificate in the form of a money order/certified check made payable to "The City of White Plains". (We do not accept personal checks), and a completed application (which you can download from this page) and mail to: Office of City Clerk, 255 Main Street, White Plains, New York 10601. Requests are usually answered the same day in which they are received.

Identification
Driver's License, Non-Drivers Identification, Passport, Military ID, Naturalization Certificate, Two (2) utility bills (for birth records only) and a police report of lost/stolen ID (for birth records only).
