Death Certificates
If the decedent died in the City of White Plains, the record is on file in our office. Death records are available to the spouse by providing a drivers license or non-drivers ID and a certified copy of the marriage license. A parent can obtain a copy of a death record with a drivers license or non-drivers ID, and a certified birth certificate of the decedent naming the parent; an adult child of a decedent needs to show a driver’s license or non-drivers ID and a certified copy of the birth certificate naming the parent. A sibling may obtain a death record by showing a birth certificate naming the same parents, and a driver's license or a non-driver's ID. Death records can be obtained by veteran administration hospitals, federal agencies for official purposes, New York State agencies for official purposes.  Others, such as a lawyer representing an estate and/or someone who can demonstrate a legal claim or an official medical need, however, may obtain a record however additional supporting documentation will be required. How to Apply
In Person
Bring the appropriate identification for the type of certificate you are requesting, along with $10 fee (either cash or money order), and a completed application for death record, which can also be provided when you come in to the office.

By Mail
Enclose a photocopy of the appropriate identification for the type of certificate you are requesting, the required fee of $10 per certificate in the form of a money order / certified check made payable to "The City of White Plains" (we do not accept personal checks), and a completed application and mail to:

Requests are usually answered the same day in which they are received.

Identification

One of the following is required:
  • Current Driver's License
  • Military ID
  • Non-Drivers Identification
  • Passport
