Birth Certificates
Copies of birth certificates are only available to the person named on the certificate (18 years or older), the person's parents, a lawful representative, or anyone who presents a court order. If you were born within the city limits of White Plains (St. Agnes Hospital, White Plains Hospital, or at home) and are over 18 years of age, you may obtain a certified copy of your birth certificate.

How to Apply
In Person
Bring the appropriate identification for the type of certificate you are requesting, along with $10 fee (either cash or money order), and a completed application for birth record which can also be provided when you come in to the office.

By Mail
Enclose a photocopy of the appropriate identification for the type of certificate you are requesting, the required fee of $10 per certificate in the form of a money order / certified check made payable to "The City of White Plains" (we do not accept personal checks), and a completed application mailed to:

Requests are usually answered the same day in which they are received.

Identification
One of the following is required:
  • Current driver's license
  • Military ID
  • Non-drivers identification
  • Passport
  • Police report of lost / stolen ID
  • Two different utility bills of applicant
